My Ceremony Waiver

Agreement Scope and Duration
I, the undersigned, being at least 18 years of age, understand and agree that this Liability Waiver and Release of Claims covers all activities, ceremonies, events, and gatherings (hereinafter referred to as “Church Activities”) organized, facilitated, or affiliated with Two Birds Church. This includes, but is not limited to:

– Sacred Ceremonies (e.g., Ayahuasca, Sapo, cacao offerings)
– Sunday services
– Potlucks, drum circles, community gatherings
– Educational workshops or retreats

This agreement will remain valid indefinitely unless I choose to revoke it in writing. Any revocation will only apply to Church Activities occurring after the date of the written revocation.

Commitment to Community and Conduct
As a participant in Church Activities, I commit to:

– Respecting the rights and beliefs of all members of the Two Birds Church community
– Honoring the sacredness of ceremonies and gatherings
– Adhering to church protocols, including those relating to safety, health, and participation
– Remaining free from drugs, alcohol, or other substances during all Church Activities
– Participating with an open heart, willingness to grow, and dedication to personal and spiritual development

I understand that Church Activities are opportunities for authentic sharing, reflection, and spiritual connection. I agree to contribute positively to this environment and to respect the facilitators, organizers, and other participants.

Health Disclosures and Responsibilities
I affirm that:

– I have truthfully disclosed all relevant health information and medications prior to participating in any Church Activity
– I do not have any known or unknown medical, physical, or mental conditions that could endanger myself or others during participation
– It is my responsibility to consult with a healthcare provider before participating in any Church Activities, including Sacred Ceremonies

I agree to follow all pre- and post-event guidelines provided by Two Birds Church, including dietary restrictions and preparation protocols for specific ceremonies. I acknowledge that my safety is primarily dependent on my own self-care and adherence to these guidelines.

Assumption of Risk and Release of Liability
I am voluntarily participating in Church Activities and fully understand that these activities may involve risks, including but not limited to:

– Emotional or psychological discomfort arising from deep personal reflection
– Physical risks associated with ceremonies or activities such as Ayahuasca or Sapo ceremonies
– Accidental injury, illness, or other unforeseen harm

In consideration of being allowed to participate in Church Activities, I hereby:

– Release Two Birds Church, its officers, directors, employees, facilitators, volunteers, and property owners from any and all liability, claims, demands, or causes of action related to my participation
– Waive any rights to pursue legal action for personal injury, emotional distress, illness, or property damage arising out of my participation
– Acknowledge that participation is voluntary and assume all associated risks

Indemnification and Hold Harmless
I agree to indemnify and hold harmless Two Birds Church, its officers, facilitators, and affiliates against any claims, costs, expenses, or liabilities, including attorney’s fees, arising out of or related to my participation in Church Activities, regardless of negligence.

Binding Effect and Entire Agreement
This agreement represents the entire understanding between myself and Two Birds Church concerning liability and participation in Church Activities. It is binding upon myself, my heirs, executors, administrators, and assigns.

Governing Law
This agreement shall be governed by and construed in accordance with the laws of the State of Texas. It applies to all participants, regardless of their state of residence.

Acknowledgment
By submitting this agreement digitally, I acknowledge that I have carefully read and fully understand the terms of this agreement. I agree to be bound by its terms voluntarily and without duress.
